STATE OF OHIO, STARK COUNTY, ss:

Name Frank J Renz applying for License for himself to be married to
Carrie Fulmer who being duly sworn, says that he is over the age
of twenty-one years, and is unmarried, that the said Carrie  is over the
age of eighteen years, and is unmarried, resides in Stark County, and
that he knows of no legal impediment to said marriage. "am not related
to her"
